Register with the Practice

Registering with Newcastle Medical Centre couldn't be easier.

First, check that you reside within the Practice Catchment Area. If you do not, you will need to complete an out of area declaration form. These registrations are assessed on a case-by-case basis. You should be aware that some of our services will be unavailable to out of area patients.

Secondly, complete all of the following

Thirdly, return the forms along with your proof of address and a copy of your photographic ID to the practice email newcastlemedical@nhs.net

Acceptable forms of ID are:

  • Passport
  • Visa
  • Student card
  • Photographic Driving Licence

If you are a student, you can register with us via the Campus Doctor site. If you choose this option, please complete the online registration forms here and send your photographic ID to the practice email within 7 days.  Failure to provide the identification in this time scale means you registration will be invalid, and you will complete the forms again.

If you are a patient who is not "ordinarily resident" in the UK, there is an information leaflet available, as you should be aware that not every person is entitled to all NHS services free of charge in England.

National Data Opt Out

Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.

You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.  There is a link under resources or a page on the website called National Data Opt Out.
